I would like Combat Stress, Ex-Services Mental Welfare Society to treat all donations I make from 6th April 2002 as Gift Aid donations, until I notify you otherwise.

Notes:

1.	a) Please notify us if you change your name or address.
	b) You can cancel the declaration at any time by notifying us.
2.	You must pay Income Tax, including tax deducted at source (e.g. building society interest), or Capital Gains Tax equal to or more than the amount we reclaim on your donations. If in the future your circumstances change and you no longer pay tax, you must cancel your declaration (see note 1).
3.	If you pay tax at the higher rate, you can claim further tax relief in your self-assessment tax return.
